About the Role

As a Home Health Physical Therapist, you'll work closely with patients who have experienced a decline in mobility, strength, and functionality due to illness or injury. Your expertise will be instrumental in helping them regain their independence and improve their overall quality of life.

Key Responsibilities
  • Provide personalized physical therapy services to patients in their homes
  • Collaborate with a multidisciplinary team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to develop and implement patient care plans
  • Conduct thorough assessments, document progress, and modify care plans as needed
  • Educate patients and their families on therapy goals, exercises, and self-care techniques
  • Coach family members on safely assisting with exercises and promoting patient independence
Requirements
  • Graduate of an accredited Physical Therapist Program and active PT license in IL
  • Minimum of 2 years of physical therapy experience, preferably in home healthcare
  • CPR certification
  • Knowledge of state and federal home health regulations
  • Excellent clinical judgment, strong interpersonal skills, and compassion
