Job Summary

We are seeking a highly motivated and compassionate Healthcare Navigator to join our team at Promise Healthcare. As a key member of our school-based health center, you will play a vital role in improving the health and well-being of our patients and the community we serve.

Key Responsibilities
  • Represent Promise Healthcare at school and community events, building relationships with patients and stakeholders.
  • Build rapport and connection with patients in an empathetic manner, ensuring their needs are met and exceeded.
  • Cross-train as a Patient Services Representative and cover for the PSR as needed, providing exceptional customer service.
  • Record enabling services provided and patient notes in the Nextgen EMR, maintaining accurate and up-to-date records.
  • Assist patients in scheduling, accessing, and utilizing clinic services, ensuring seamless transitions and optimal care.
  • Screen patients for Social Determinants of Health Issues (SDOH) and make service referrals as appropriate, addressing the root causes of health disparities.
  • Participate in the School Clinic and Community Coalition, fostering collaboration and coordination among stakeholders.
  • Assist with grant reporting and needed tracking and record keeping, ensuring compliance and accountability.
  • Assist with outreach to patients and potential patients through various channels, promoting services and building relationships.
  • Attend school and community events to promote services, engage with stakeholders, and build partnerships.
  • Work with Marketing and Patient Access staff to promote school health center services, increasing utilization and awareness.
  • Follows HIPAA policies and procedures, maintaining confidentiality and adhering to regulatory requirements.
  • Assesses patient needs to customize appropriate internal and external resources, addressing the unique needs of each patient.
  • Works with individuals from diverse backgrounds to reduce cultural and socio-economic barriers, promoting equity and access to care.
Requirements
  • Ability to think critically and make decisions based on information provided.
  • Maintain professionalism with patients, co-workers, staff, and referring agencies.
  • Self-starter and independent worker, able to work effectively with minimal supervision.
  • Effective communication skills by telephone, in person, and virtually.
  • High school diploma required, associate degree preferred.
  • Experience with Medicaid or Marketplace insurance enrollment a plus, with willingness to train.
  • Willingness to complete Community Health Worker training, enhancing skills and knowledge.
  • Ability to attend occasional community events on evenings or weekends, as needed.
Work Environment

This is a long-term temporary position, grant-funded through June 2025. The work may require some early morning hours, evenings, and weekends to participate in school and community events. This position reports to the Vice President of Strategy and Development.
