We are seeking a skilled Physical Therapist to join our team at LifeCircles PACE in Holland. As a Physical Therapist, you will evaluate participants aged 55 or older and assess their mobility level and other functional abilities.
Key Responsibilities- Evaluate participants and assess their mobility level and other functional abilities
- Collaborate with the therapy team and other interdisciplinary team members
- Provide and recommend therapeutic exercise, gait training, and modalities
- Plan and develop group exercise programs for maintenance therapy
- Conduct initial and periodic assessments and develop a specific plan of care
- Assess the need for adaptive and/or durable medical equipment, orthotics, and prosthesis
- Recommend physical therapy services to the Interdisciplinary Team
- Plan, develop, and implement a plan of therapy to include exercise, treatments, and modalities
- Assess newly issued orthotic shoes, braces, and prosthetics for appropriateness and proper fit
- Provide treatment in appropriate settings to meet participant care needs
- Participate in discharge planning and assist with DME management
- Current, unencumbered, active license to practice as a Physical Therapist in the State of Michigan
- Comfortable in a self-directed environment
- Proficient in computer use, including Excel, Microsoft Office, Outlook, and electronic medical records
