Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled Home Health Physical Therapist to join our team in Fairview, New Jersey.

Job Summary:

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our clients in their homes. As a Home Health Physical Therapist, you will work independently to assess, plan, and implement individualized care plans to help clients achieve their rehabilitation goals.

Responsibilities:
  • Conduct thorough assessments of clients' physical abilities and develop personalized care plans to address their needs.
  • Implement evidence-based interventions, such as manual therapeutic exercises, gait training, and balance activities, to promote clients' physical function and mobility.
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordinate services as needed.
  • Provide education and training to clients and their families on proper techniques and strategies for maintaining physical function and preventing complications.
  • Document all interactions, interventions, and progress in clients' medical records.
Requirements:
  • A current NJ Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
