The Licensed Practical Nurse is a vital member of our team, responsible for delivering high-quality patient care in the comfort of their own homes. As a skilled nursing care provider, you will work closely with our Registered Nurse teammates to develop and implement individualized care plans that meet the unique needs of each patient.

Key Responsibilities:

  • Deliver quality patient care in compliance with physician orders, under the direction of our Registered Nurse teammates.
  • Recognize changes in patient needs and responses, and implement care to prevent risk or reduce risk.
  • Provide education to clients and their family members based on their specific needs.
