Physician Assistant

Theoria Medical is seeking a skilled Physician Assistant to join our team in providing high-quality, patient-centered care to residents in skilled nursing facilities (SNFs) and primary care settings. As a Physician Assistant, you will be responsible for rounding and managing the clinical care of the population at the assigned SNF, prescribing medications and treatments, and providing comprehensive primary care services.

• Rounding and managing the clinical care of the population at the assigned SNF
• Prescribing medications and treatments for residents residing at SNFs
• Providing high-quality, patient-centered, comprehensive primary care services to patients residing in SNFs
• Timely documentation of all patient care/encounters
• Availability for phone/secure messaging consultations with facility nursing staff

• Reliable transportation required
• Timely attendance is imperative
