Job Description

This full-time position involves working closely with a multidisciplinary team to assess patients' needs and develop personalized care plans. Your primary responsibility will be to assist patients and their families in navigating the healthcare system, addressing any concerns or challenges they may face.

  • Collaborate with healthcare professionals to develop and implement patient-centered care plans.
  • Provide education and support to patients and their families on health-related issues.
  • Assess patients' physical and emotional needs and identify opportunities for improvement.
Required Skills and Qualifications

To be successful in this role, you will need:

  • A Master's degree in Social Work from an accredited institution.
  • A current Social Work license in the state of South Dakota or Iowa, Nebraska, Minnesota, or North Dakota.
  • Excellent communication and interpersonal skills.
  • Able to work effectively in a fast-paced environment.
