**Job Summary:**
We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Palliative Care Specialist. As a key member of our interdisciplinary team, you will provide high-quality patient care, develop and implement individualized care plans, and collaborate with healthcare professionals to ensure seamless care transitions.
Key Responsibilities:
- Provide comprehensive assessments of patients' physical, emotional, spiritual, and psychosocial needs.
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare professionals.
- Administer medications, including controlled substances, according to physician orders and policy.
- Communicate effectively with patients, families, and healthcare professionals to ensure timely and accurate information exchange.
- Document patient care, including variances in patient condition, interventions, and outcomes.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Supervise Certified Nursing Assistants (CNAs) and Licensed Practical Nurses (LPNs).
Requirements:
- Current and active Florida RN license.
- Must be Certified Hospice and Palliative Registered Nurse (CHPN) within 12 months.
- Current and valid Florida driver's license and automobile insurance.
- Current CPR certification required.
- Minimum of two years RN experience where physical assessment skills were utilized.
- Prior experience in oncology, home health, Hospice, or palliative care setting is preferred.
Working Conditions:
- Standing.
- Sitting.
- Walking.
- Lifting (raising or lowering without mechanical aids) 50 lbs.
- Pushing or pulling.
- Carrying objects.
- Climbing (ascending or descending stairs or ladders).
- Crouching (bending at the knees).
- Kneeling (resting on hands and knees).
- Crawling (moving about on hands and knees).
- Stooping (bending at the waist).
- Reaching (extending hands and arms in any direction).
- Handling (seizing, holding, grasping, or turning objects).
- Typing, picking, pinching, or otherwise working with fingertips.
- Using arm muscles frequently.
- Using leg muscles frequently or for extended periods.
- Transferring patients: transfer from bed to chair, transfer from chair to bed, and transfer from chair to toilet.
