Job Summary

Key Responsibilities
  • Provide accurate and comprehensive assessments, diagnoses, and treatment plans for patients
  • Collaborate with the clinical care team to ensure seamless care coordination and communication
  • Develop and implement patient-centered care plans, incorporating evidence-based practices and guidelines
  • Manage chronic disease conditions, including diabetes, hypertension, and asthma
  • Provide health education and counseling to patients and families on disease management and prevention
  • Document patient care in electronic health records, ensuring accuracy and timeliness
Requirements
  • Master's degree in Family Nurse Practitioner with current National Board Certification and State of Employment license to practice in the Advanced Practice role
  • Minimum 1 year experience as a Nurse Practitioner, with experience in Family medicine or primary care practice preferred
  • Active Advance Practice Registered Nurse, Nurse Practitioner license in good standing
  • Current National Board Certification in Family Practice
Preferred Qualifications
  • Working knowledge and understanding of quality measurement in the management of chronic disease conditions
  • Self-motivated, prioritizes and solves problems, takes initiative, and advocates for patients and practice
  • Effective verbal, written, and electronic communication skills
  • Outstanding organizational skills and ability to multi-task
