The Licensed Physical Rehabilitation Specialist (PT, DPT, RPT) in Home Health is accountable for assessing and evaluating patient care requirements related to mobility, endurance, pain, and other health conditions.

Based on thorough assessments, the Specialist collaborates to formulate a treatment strategy, implements interventions aimed at improving the patient's overall health, and monitors the patient's progress.

Key Responsibilities:

  • Evaluate the patient's functional capabilities and physical therapy requirements.
  • Assist physicians in developing comprehensive therapy plans.
  • Observe and communicate the patient's response to treatment and any changes in their condition to the supervising nurse or physician.
  • Administer treatments to alleviate pain, restore function, and maintain optimal functioning according to established standards.
  • Educate and inform all members of the healthcare team, patients, and their families/caregivers about the care plan, functional limitations, and progress towards goals.

Licensure Requirements:

  • Current Physical Therapy licensure in the state of practice
  • Current CPR certification
  • A valid driver's license and vehicle insurance, along with access to a reliable vehicle
