About the Position

We're seeking a skilled and compassionate Nurse Practitioner to join our team. As a Geriatric Care Specialist, you'll be responsible for providing primary care to seniors in their homes. You'll work collaboratively with our interdisciplinary team to deliver high-quality, patient-centered care. This is a full-time position with a regular schedule, and you'll have the opportunity to work from home and travel to other locations as needed.

Responsibilities
  • Provide primary care to seniors in their homes, including routine check-ups, urgent care, and chronic disease management
  • Collaborate with our interdisciplinary team to deliver comprehensive care
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination
  • Stay up-to-date with the latest medical research and best practices in geriatric care
Requirements
  • Current licensure as a Nurse Practitioner in the state of practice
  • Experience working with geriatric patients and their families
  • Strong communication and interpersonal skills
