Nurse Care Manager

Care Management RN Opportunity
We are seeking a skilled and compassionate Registered Nurse to join our team as a Care Management RN. In this role, you will be responsible for the care management of patients with acute and chronic medical conditions, assessing the care needs of vulnerable populations, and collaborating with healthcare providers to develop and implement effective care plans.
Key Responsibilities:
- Performs assessment and evaluation of potential care management patients per client contractual obligations and/or state requirements.
- Collaborates with healthcare providers and ancillary service providers to validate the appropriate treatment plan and identify educational opportunities.
- Provides ongoing education and communication with healthcare providers to ensure appropriateness of care.
- Evaluates treatment adherence and reevaluates the patient's commitment to making necessary changes.
- Documents per contractual requirements related to individual member status, and/or medical necessity review, and negotiation.
Requirements:
- Current and unencumbered RN License.
- Graduation from a nationally accredited nursing program.
- Candidates must be eligible to obtain additional state licensure through the endorsement process after hire.
- Four-year degree in healthcare or related field, and/or equivalent training, and/or experience.
- Minimum 1 year experience in human services, and/or 3 years clinical practice experience.
- Oncology Certified Nurse (OCN) is preferred.
