Job Summary

We are seeking a Licensed Practical Nurse (LPN) to join our skilled nursing team at Advanced Center for Nursing and Rehabilitation in New Haven, CT.

About the Role

This is a full-time and part-time position available, with competitive pay rate and benefits. As an LPN, you will be responsible for providing high-quality patient care, administering medications, and collaborating with our interdisciplinary team to ensure excellent outcomes.

Key Responsibilities:

  • Ordering prescribed medications, supplies, and equipment as needed
  • Recording health details of residents, including vitals and temperature
  • Carrying out direct care to residents based on the care plan, in accordance with facility policy and nursing standards of practice
  • Administering medications and treatments to residents as needed
  • Supervising Certified Nursing Assistants
  • Performing assigned duties, including wound care, treatments, and procedures
  • Maintaining confidentiality and rights of residents, family members, employees, and facility information in compliance with HIPAA policies
Requirements

To be considered for this role, you must have a valid CT State LPN license and possess excellent leadership and communication skills. Long-term care experience is preferred.
