Job Summary

We are seeking a skilled LPN Nurse to join our team at a long term care, memory care, short-term rehabilitation, and senior independent living facility. As an LPN Nurse, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to ensure the best possible outcomes for our residents.

Key Responsibilities
  • Administer medications and treatments in accordance with accepted standards
  • Provide safe and efficient personal care to residents
  • Use recommended safety devices and follow infection control standards
  • Answer call lights and residents' requests for assistance
  • Adhere to residents' rights at all times
  • Document medications and treatments administered, and care provided
  • Assist with orientation of new employees on the unit
  • Participate in resident care planning, updating care plans and aide assignments
  • Assess residents, document findings, and report to the physician; Transcribe physicians' orders
  • Communicate resident change of condition to resident sponsors
  • Destroy and document discontinued medications
  • Maintain correct controlled substance numbers during shift
  • Complete required documentation of nursing notes, transfer forms, and other records
  • Assist with operation of the unit, including the monthly change over of records
  • Complete and distribute nurse aide assignments
  • Give report to on-coming shift
  • Schedule and assist with physician, optometrist, podiatrist, dentist appointments
  • Administer and record employee health testing results
Requirements
  • Indiana Professional License or License from an NLC participating state required at time of submission
  • Proficiency in computer skills and ability to learn other programs used in maintaining electronic medical records
  • Hands On - American Heart Association CPR and BLS Certification required
  • Dementia 6 hour certification is required prior to start
  • LTC experience required
  • Preferred: EMR, Matrix Care experience, Experience with Memory Care, Experience with short-term rehab
Physical Demands

The ability to safely and successfully perform the essential job functions consistent with the ADA and other federal, state, and local standards. Sedentary work that involves sitting or remaining stationary most of the time with occasional need to move around the office to attend meetings, etc. Ability to conduct repetitive tasks on a computer, utilizing a mouse, keyboard, and monitor.
