Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at Actalent. As a LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.

Job Summary:

The Licensed Practical Nurse will be responsible for planning, directing, and coordinating the activities of nursing personnel within assigned areas. This includes reviewing care plans, ensuring continuity of care, and maintaining accurate records and reports.

Key Responsibilities:
  • Plan, direct, and coordinate nursing activities to ensure high-quality patient care.
  • Review and revise care plans as needed to ensure they meet the identified nursing needs of patients.
  • Ensure continuity of care by communicating with supervisors and staff through written reports and verbal communication.
  • Maintain accurate records, files, and reports as directed.
Requirements:
  • Active LPN license from the State of New York.
  • At least 6 months of experience working as a LPN.
  • Experience working in a nursing home setting is preferred.
