Job Summary

We are seeking a highly motivated and compassionate Direct Support Professional II to join our team at Evergreen Life Services. As a key member of our support services team, you will play a vital role in ensuring the health, safety, and well-being of individuals served.

Key Responsibilities
  • Health and Welfare: Ensure the health and welfare of individuals served, including administering medication as allowed by state guidelines.
  • Medication Administration: Assist individuals served with medication and treatment plans, counting and dispensing medications as outlined in state guidelines.
  • Training and Development: Train individuals served in basic housekeeping tasks, such as making a bed, cleaning the floor, and other essential skills.
  • Housekeeping and Maintenance: Clean the living area, assist with laundry, and report maintenance needs to ensure a safe and healthy environment.
  • Financial Management: Assist individuals served with money management, including budgeting, banking, and shopping.
  • Daily Life Activities: Assist individuals served with daily life activities, such as meal preparation, reading usage, use of telephone, and community services.
  • Treatment Planning: Participate in the development and implementation of active treatment plans, nutritional, behavioral, and physical support plans.
  • Transportation: Accompany and transport individuals served to medical appointments, church services, recreation areas, work, and shopping as requested.
  • Food and Non-Food Purchases: Make food and non-food purchases for the home when requested.
  • Meal Preparation: Prepare meals for individuals served.
  • Maintenance and Grounds: Ensure the maintenance of the home and grounds is done as requested.
  • Documentation: Complete all required documentation, including daily notes, incident reports, and medical information.
  • Behavioral Programs: Implement behavioral and other programs as trained and requested.
  • Goal Achievement: Assist individuals served in achieving their goals outlined in their plan of care document.
  • Behavioral Triggers: Identify potential behavior triggers and defuse or redirect them as needed to ensure the safety and well-being of all involved.
  • Transportation Services: Provide transportation for individuals served in the company or personal vehicle as requested.
  • Accidents and Incidents: Report accidents/incidents and suspected cases of abuse or neglect of individuals served per Evergreen policies and procedures.
  • Vocational Opportunities: Provide instruction to individuals served involved in work activities associated with vocational opportunities.
Qualifications and Experience
  • Communication Skills: Effectively read, write, and communicate verbally in English, and must be able to read and understand instructions, perform record-keeping, and write reports.
  • Driver's License: Valid driver's license, which meets Evergreen requirements for insurance purposes and proof of liability coverage if a vehicle is owned, is preferred.
  • State Requirements: Meet and maintain all requirements of the applicable state agency(ies), including good standing with the state abuse and neglect registry.
  • Background Check: Submit to and pass a drug screen and criminal background check.
  • Computer Skills: Ability to utilize basic computer skills, such as data entry, approving timesheets, etc.
  • Medication Administration Certification: Successful completion of appropriate state-required medication administration certification and up-to-date training.
  • Age Requirement: Must be at least 18 years of age (non-driver) or 21 years of age (driver).
Physical Requirements
  • Physical Activity: Constantly moves about to coordinate work.
  • Lifting and Positioning: Regularly moves and positions objects weighing up to 50 pounds while assisting individuals served in home, workshop, or job site.
  • Physical Assistance: Regularly assist individuals served physically by lifting and positioning them as appropriate.
  • Alertness: Constantly alert and aware of the needs of individuals served.
  • Infectious Conditions: Occasionally exposed to viruses and infectious conditions.
