Vocational Nurse

At Varco Home Health and Hospice, we are seeking a skilled and compassionate LVN to join our team of healthcare professionals. As a Home Health Licensed Vocational Nurse, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

• Conduct Comprehensive Health Assessments: Evaluate patients' vital signs, medication adherence, and overall health status to develop personalized care plans.
• Develop and Implement Care Plans: Collaborate with the healthcare team to create individualized care plans that meet patients' needs and goals.
• Administer Medications and Treatments: Safely administer medications, treatments, wound care, and medical procedures following prescribed protocols and care plans.
• Monitor and Document Patient Progress: Regularly monitor patients' condition changes and document updates to the healthcare team.
• Education and Support: Educate patients and their families on disease management, medication adherence, and lifestyle adjustments to promote wellness.
• Interdisciplinary Collaboration: Work collaboratively with various healthcare professionals, including physical therapists, occupational therapists, and social workers, to ensure a holistic approach to patient care.

• Current LVN License: Must be licensed as an LVN in the state of Texas.
• Home Health Experience: Proven experience as an LVN, preferably in a home health setting.
• Clinical Assessment and Patient Care: Proficiency in clinical assessment and patient care.
• Communication and Interpersonal Skills: Strong communication and interpersonal skills.
• Independence and Teamwork: Ability to work independently and as part of a team.
• Valid Driver's License: Must have a valid driver's license and reliable transportation.
