Care Transition Coordinator RN Job Description

LHC Group is seeking a skilled Care Transition Coordinator RN to join our team in Jackson, Mississippi.

Job Summary:

We are looking for a compassionate and organized RN to facilitate seamless transitions for patients discharging from a facility setting to the care of an LHC Group hospice agency.

Responsibilities:

  • Facilitate patient transitions from facility to hospice care
  • Assist with preparing for and accepting care of patients post-discharge
  • Execute contracts for facility-based services for hospice patients
  • Explain hospice services and agency procedures to patients and their families
  • Participate in bi-weekly IDG meetings to discuss account updates and progress towards agency growth strategies

Requirements:

  • 1 year of home health experience or 1 year of hospital case management experience
  • 1-3 years of medical marketing experience preferred
  • Current RN or LPN or SW or PT licensure in state of practice
  • Reliable means of transportation and current driver's license and auto insurance
  • Excellent verbal and written communication skills
  • Excellent organizational skills and ability to complete competing priorities
  • Thorough understanding of home health qualifying criteria and coverage guidelines
  • Proficient computer skills
