Key Responsibilities:

Implement individualized care plans for clients and document completed tasks. Aid clients with personal care tasks such as bathing and dressing. Assist with household chores, meal preparation, and laundry services. Provide transportation for clients to medical appointments, grocery shopping, and other essential errands. Support clients in the self-administration of medications. Monitor and report any changes in the client's health status. Uphold confidentiality at all times due to access to sensitive information. Maintain consistent attendance and flexibility to meet departmental needs. Adhere to all Medicare, Medicaid, and HIPAA regulations and standards. Follow all organizational policies, procedures, and guidelines. Perform additional duties as required.

Position Requirements & Qualifications:

Must be at least 18 years old. Successful completion of the Agency qualifying examination. Ability to pass a criminal background check. CPR / First Aid certification if required by state or program. Compassionate and nurturing disposition with a commitment to caring for others.
