Job Summary

We are seeking a highly skilled Registered Nurse to join our Virtual Care team. As a Virtual Registered Nurse, you will be responsible for providing high-quality patient care through a computerized and/or digital platform. This is a full-time, night shift position with a 7pm-7:30am schedule, including weekend and holiday rotations.

Responsibilities
  • Assess, plan, implement, coordinate, monitor, and evaluate patient care plans from admission to discharge.
  • Utilize clinical knowledge, critical thinking skills, and the principles of case management and adult education to coordinate, implement, and support patient care plans.
  • Ensure seamless, effective, and efficient transitions of care across the continuum.
  • Monitor quality metrics specific to the department and ensure complete and accurate documentation in the patient record.
  • Perform the roles of mentor, clinician, educator, consultant/collaborator, and navigator.
Requirements
  • Graduation from an accredited professional school of nursing.
  • Valid RN licensure authorized in the applicable state(s) of practice/employment.
  • Valid driver's license where required by assignment.
  • Minimum eight (8) years of experience in health condition management, which must include adult condition management.
  • Electronic Medical Record (EMR) experience.
