We are seeking a skilled and compassionate RN Case Manager to join our team at Trinity Health. As a key member of our healthcare team, you will play a vital role in ensuring the seamless transition of patients from acute care to community-based care.
Key Responsibilities- Conduct thorough assessments of patients' healthcare needs and develop individualized care plans to meet those needs.
- Collaborate with interdisciplinary teams to coordinate patient care and ensure timely and effective transitions.
- Conduct utilization reviews to identify opportunities for cost savings and quality improvement.
- Develop and maintain relationships with community-based providers to ensure seamless transitions of care.
- Stay up-to-date on industry trends and best practices in case management and utilization review.
- Current GA RN license
- Graduate of an accredited school of nursing, BSN recommended
- Minimum 2 years' experience in nursing and 1 year in case management, utilization management, or discharge planning
- Certification in case management preferred
