Job Title: Physical Therapist Travel Assignment

Fusion Medical Staffing-Therapy is seeking a skilled Physical Therapist to join our team for a travel assignment in Portage, Michigan.

Job Description:

We are looking for a dedicated and compassionate Physical Therapist to provide high-quality patient care in a home health setting. The ideal candidate will have experience working with patients in a home health environment and be able to work independently with minimal supervision.

Responsibilities:
  • Provide physical therapy services to patients in their homes
  • Develop and implement individualized treatment plans to meet the unique needs of each patient
  • Conduct thorough assessments and evaluations to identify patient needs and goals
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
  • Document patient progress and outcomes in a timely and accurate manner
Requirements:
  • Current licensure as a Physical Therapist in the state of Michigan
  • One to three years of experience as a Physical Therapist in a home health setting
  • Ability to work independently with minimal supervision
  • Excellent communication and interpersonal skills
  • Ability to lift and move patients as needed
