Outpatient Care Nurse

Position Overview:
The Outpatient Care Nurse is accountable for delivering nursing services to patients, adhering to the established Baystate Health Standards of Care. This role involves administering treatments and observing patient reactions to therapeutic measures. The nurse applies comprehensive knowledge of the nursing process in collaboration with patients and their families, ensuring a holistic approach to care.
In this capacity, the nurse supervises and delegates tasks to other healthcare team members as appropriate, ensuring the effective execution of the patient care plan. Acting as a patient advocate, the nurse operates within ethical and legal nursing frameworks, promoting the best interests of patients and their families.
Key Responsibilities:
1) Clinical Assessment and Planning - Independently evaluates, formulates, and executes care plans for patients with complex medical and psychosocial conditions.
Utilizes advanced assessment skills to provide quality triage and patient care, ensuring adherence to safety protocols and quality assurance measures.
Develops proficiency in identifying behavioral patterns and clinical symptoms to enhance care protocols and treatment strategies.
Mentors and guides clinical staff, ensuring ongoing competency development to meet safety and quality benchmarks.2) Innovation in Clinical Practice - Advocates for and participates in the development and implementation of evidence-based protocols and treatment plans.
Promotes patient safety and utilizes safety reporting systems to create actionable improvement plans.
Identifies research opportunities and collaborates with clinical staff to assess the feasibility of studies.
3) Patient and Family Engagement - Evaluates barriers to care, considering social and cultural factors that may affect patient outcomes, and devises effective solutions.
Encourages patient and family involvement in the decision-making process regarding care.
4) Education and Transition Management
Recognizes and addresses the educational needs of patients and their families.
Anticipates resource requirements and care needs during transitions between care settings.
5) Interdisciplinary Collaboration
Effectively collaborates with the healthcare team to coordinate patient-centered care, optimizing resource utilization for the best patient outcomes.
Serves as a knowledgeable resource for staff, providing clear and detailed guidance.6) Effective Communication - Adapts communication styles to meet the needs of diverse patients and families.
Handles challenging conversations with professionalism, ensuring clarity and respect.
7) Leadership and Mentorship - Provides education on equipment use and quality assurance standards.
Takes on responsibilities as needed and mentors students and residents.
Qualifications:
1) Minimum of two (2) years of clinical experience as a Registered Nurse.
2) Any exceptions must receive approval from the Chief Nursing Officer.
