Job DescriptionAs a Home Health Physical Therapist Assistant with Interim HealthCare of New Hampshire, you will play a vital role in helping patients regain their strength and manage their pain in the comfort of their own homes. Our team of healthcare professionals is dedicated to providing personalized, home-based therapy that is tailored to each patient's unique needs and goals.

As a Home Health Physical Therapist Assistant, your primary responsibility will be to provide high-quality physical therapy services to patients in their homes. This will involve conducting assessments, developing and implementing individualized care plans, and educating patients and their families on proper exercise techniques and self-care strategies.

In this role, you will have the opportunity to work with a diverse range of patients and develop a deep understanding of their unique needs and challenges. You will also have the chance to work collaboratively with other healthcare professionals, including nurses, occupational therapists, and speech-language pathologists, to ensure that each patient receives comprehensive and coordinated care.

Key Responsibilities:

* Provide home-based physical therapy services to patients who have lost mobility, strength, and functionality due to illness or injury
* Conduct assessments and develop individualized care plans to meet each patient's unique needs and goals
* Educate patients and their families on proper exercise techniques and self-care strategies
* Collaborate with other healthcare professionals to ensure comprehensive and coordinated care
* Maintain accurate and detailed records of patient progress and care plans

Requirements:

* Active PTA license in the State of New Hampshire
* Minimum of 1 year of PTA experience, ideally in home healthcare
* CPR certification
* Up-to-date on all State of New Hampshire required vaccinations
* Knowledge of state and federal home health regulations
* Good clinical judgment, strong interpersonal skills, dexterity, and compassion
