Job Summary

We are seeking a highly skilled and compassionate Nurse Practitioner or Clinical Nurse Specialist to join our team at One Health. As a key member of our healthcare team, you will be responsible for delivering high-quality patient care and promoting health and wellness in our communities.

Key Responsibilities
  • Triage patients, collecting vital signs, reviewing health history, and medication reconciliation, as well as PHQ-2 and PHQ-9, GAD 7 information.
  • Document data into Electronic Health Record in accordance with established Patient Centered Medical Home (PCMH), Uniform Data System (UDS) and Meaningful use measures.
  • Provide basic education information to patients and family, focusing on self-care appropriate to their age and identified learning considerations and needs.
  • Administer medications per provider orders.
  • Anticipate and prepare examination room with equipment and supplies required for the visit.
  • Assist provider with examination and procedures per request.
  • Perform lab testing, including quality controls, maintain all laboratory/diagnostic test logs, and report all critical lab values immediately.
  • Administer vaccines to patients in accordance with current vaccine schedules.
  • Participate in huddle with primary care team.
  • Handle telephone encounters and medication refill requests for primary care team.
  • Effectively use the electronic medical record for charting patient nursing visits and telephone contacts.
  • Keep client medical records updated for reviews, audits, and contact purposes.
  • Participate in quality assurance/quality improvement projects as needed or requested.
  • Work as a team member with the primary care teams.
  • Communicate clearly, concisely, and accurately, creating a working climate that provides growth and job satisfaction of personnel.
  • Use professional courtesy when communicating with patients, team members, other co-workers, and community members.
  • Contact patients regarding lab results.
  • Identify patients for specialized treatment services, and groups.
  • Identify health literacy needs for patients and provide proper health education materials.
  • Upon request, provide brown bag medication review with patients.
  • Develop and/or identify appropriate patient education materials for use with patients and families.
  • Assist with outreach activities as needed. This may include staffing health fairs, taking blood pressures, blood glucose testing.
  • Work collaboratively with other health center staff in meeting accreditation and other regulatory and/or funding requirements.
Requirements
  • Education/Certificates/Licenses:
    • Required:
      • Degree in nursing from an accredited nursing program
      • Current license in the State of Wyoming
      • BLS Certification upon hire
      • Valid driver's license
  • Experience:
    • Preferred:
      • Three (2) to five (5) years' in clinical setting of primary care (RN)
      • Three (3) to five (5) years' in clinical setting of primary care (LPN)
