Job Summary:The ideal candidate will possess a current Connecticut license and at least two years of recent experience as a Certified Occupational Therapist Assistant in LTC SNF settings. The successful candidate will be responsible for delivering exceptional care and making a significant impact in the healthcare community.
Responsibilities:- Provide high-quality patient care and services
- Collaborate with healthcare professionals to develop and implement patient care plans
- Conduct thorough assessments and evaluations to identify patient needs
- Develop and implement individualized patient care plans to meet specific needs
- Provide education and training to patients and families on proper care and treatment
- Current Connecticut license
- At least two years of recent experience as a Certified Occupational Therapist Assistant in LTC SNF settings
- Current BLS and/or ACLS and/or Specialty Certifications
- 2 current Clinical References
- Physical and TB Records
- Tdap/Flu vaccinations and Titers
- MMR/Hep B/Varicella or vaccination records
- Ability to pass background/urine drug screening
