Job Summary:

The ideal candidate will possess a current Connecticut license and at least two years of recent experience as a Certified Occupational Therapist Assistant in LTC SNF settings. The successful candidate will be responsible for delivering exceptional care and making a significant impact in the healthcare community.

Responsibilities:
  • Provide high-quality patient care and services
  • Collaborate with healthcare professionals to develop and implement patient care plans
  • Conduct thorough assessments and evaluations to identify patient needs
  • Develop and implement individualized patient care plans to meet specific needs
  • Provide education and training to patients and families on proper care and treatment
Requirements:
  • Current Connecticut license
  • At least two years of recent experience as a Certified Occupational Therapist Assistant in LTC SNF settings
  • Current BLS and/or ACLS and/or Specialty Certifications
  • 2 current Clinical References
  • Physical and TB Records
  • Tdap/Flu vaccinations and Titers
  • MMR/Hep B/Varicella or vaccination records
  • Ability to pass background/urine drug screening
