About the Role

We are seeking a skilled and compassionate Home Health Nurse to join our team at Stonerise Home Health New Martinsville. As a Home Health Nurse, you will play a vital role in providing high-quality care to patients in the comfort of their own homes.

Key Responsibilities
  • Conduct thorough assessments of patients to determine their care needs and develop personalized care plans.
  • Supervise and direct the implementation of care plans by Licensed Practical Nurses (LPNs) and Home Health Aides.
  • Develop and implement patient education plans to promote health and wellness.
  • Evaluate and revise care plans as patient needs and status change.
  • Administer medications and treatments as prescribed by physicians.
  • Coordinate discharge planning to ensure a smooth transition for patients.
  • Act as a Case Manager for assigned caseloads, coordinating patient care and communicating with physicians and other healthcare professionals.
Requirements
  • Graduate of an accredited school of nursing.
  • Active RN licensure.
  • One to two years of recent acute care experience in an institutional setting.
  • Bachelor's degree with one year of home health care experience preferred.
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements.
