Job Overview

As a Physical Therapist with CenterWell Primary Care, you will play a vital role in delivering personalized home care services to patients managing chronic conditions or recovering from injury or illness. Our skilled therapy team works closely with nurses, occupational therapists, speech-language pathologists, and home health aides to provide comprehensive care that addresses the physical, emotional, and social well-being of our patients.

Required Skills and Qualifications
We are seeking a highly skilled and compassionate Physical Therapist who possesses strong organizational and communication skills. The ideal candidate will have a degree from an accredited Physical Therapy Program and current licensure as a Physical Therapist. A minimum of one year of experience in physical therapy is preferred, with Home Health experience being a plus. Current CPR certification is also required.

Scheduled Weekly Hours
