About the Role:
- As a Registered Nurse, you will provide high-quality patient care and support to individuals in their homes, focusing on hospice care and case management.
- You will work closely with our team to develop and implement personalized care plans, ensuring the best possible outcomes for our patients.

Responsibilities:
- Conduct home visits to assess patient needs and develop care plans in collaboration with our team.
- Administer medication, wound care, treatments, and procedures as prescribed by physicians.
- Monitor patient conditions, reporting changes to our Clinical or Client Services Manager.
- Follow up with and execute doctors' orders, ensuring accurate documentation of patient care.
- Perform client assessments as necessary and provide case management and coordination services.
Qualifications:
- A current license as a Registered Nurse in Delaware.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care experience is strongly preferred, but not required.
- Graduation from an accredited and approved nursing program.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills and solid computer skills, with prior experience with electronic medical records (EMR) preferred.
