Job Overview

POSITION TITLE: Nurse Practitioner - Primary Care Services Specialist (NP) Non-Exempt Role

SALARY: $100,000 - $115,000

REPORTS TO: Chief Clinical Officer and Medical Director

KEY RESPONSIBILITIES: Lifeline Connections is a community-focused behavioral health organization dedicated to delivering confidential and compassionate support to individuals facing substance use and/or mental health challenges. Our Primary Care Services model is designed to provide short-term outpatient primary care, essential mental health services, and, when necessary, addiction health services. We serve as a vital link for patients until they can access long-term primary care and/or mental health and addiction treatment. This role may also involve providing training and education for Family Medicine medical students and residents.

Responsibilities include:

  1. Administer interim care and medications (if indicated) while screening for chronic conditions such as hypertension, diabetes, and hyperlipidemia.
  2. Address minor injuries including strains, sprains, and uncomplicated extremity fractures and dislocations within the physician's scope of practice.
  3. Manage superficial minor lacerations, abrasions, and first and second-degree burns within the provider's scope of practice.
  4. Facilitate medication refills for chronic medical and mental health issues (in collaboration with mental health providers).
  5. Conduct evaluations, examinations, and develop individualized treatment plans for acute and chronic conditions, as well as minor injuries.
  6. Provide Hepatitis C evaluations and treatment (training available).
  7. Conduct MAT evaluations, treatment planning, and medication management as needed (training available).
  8. Order and interpret diagnostic tests, including labs and imaging.
  9. Collaborate with other healthcare providers, including mental health professionals, nurses, medical assistants, and office staff.
  10. Offer counseling and guidance to patients on health maintenance topics such as nutrition, risk factor prevention, mental health referrals, self-care measures, family planning, and STD prevention.
  11. Identify and refer patients to higher levels of care as necessary.
  12. Assist the medical team in coordinating care received by patients from various providers and advocate for patients within the healthcare system.
  13. Participate in withdrawal management rounds as needed.
  14. Commit to providing culturally sensitive care.
  15. Support health education initiatives for individual patients or groups as required.
  16. Provide staff development education as requested by the Program Director and other departments.
  17. Serve as a preceptor for medical students and residents during their rotations.
  18. Engage in ongoing education and specialized training to maintain high-quality care for the clinic's patient population.
  19. Attend staff and provider meetings.
  20. Document patient care clearly and appropriately in accordance with medical standards.
  21. Notify Human Resources of any changes in licensure, DEA credentials, board certification, or Medicare provider status.
  22. Perform additional duties as assigned.

POSITION REQUIREMENTS:

  1. Valid Washington State Department of Health medical license required.

Qualifications:

  1. Preferred Board certification or Board eligibility in Family Practice or Internal Medicine or equivalent training.
  2. Board certification in Addiction Medicine preferred but not mandatory.
  3. Current CPR certification.
  4. Experience working with underserved populations, including those experiencing homelessness and addiction, preferred.
  5. Experience with MAT medication management is a plus but not required.
  6. Possess a current DEA credential with X waiver.
  7. Ability to maintain patient confidentiality.
  8. Strong verbal and written communication skills.
  9. Reliable attendance.
  10. Ability to collaborate effectively with others.

GUIDELINES:

This position operates in compliance with Washington Administrative Codes, federal regulations governing confidentiality, and all Lifeline Connections policies and procedures. The incumbent must maintain certification or qualification standards as outlined in relevant RCWs and WACs.

COMPLEXITY:

The role involves providing individual therapy, group therapy, case management, and diagnostic services to clients with diverse backgrounds and varying socioeconomic statuses, often dealing with complex social, behavioral, and psychological issues. Strong clinical skills, sound judgment, and creativity are essential for delivering effective treatment services.

PERSONAL CONTACT:

Interactions occur with treatment team members, clients, their significant others, board members, and representatives from various community agencies focused on treating substance abuse and psychiatric disorders. All interactions are conducted in accordance with federal and state regulations regarding client confidentiality.

PHYSICAL DEMANDS:

While performing essential job functions, the employee is regularly required to sit, speak clearly, hear, and use hands and fingers for writing and typing. Occasional standing, walking, reaching, stooping, kneeling, crouching, or lifting up to 15 lbs may be required.

WORKING ENVIRONMENT:

Work hours are primarily indoors in office settings or meeting rooms, with occasional outdoor supervision of activities, visits to community agencies, and participation in staff retreats and development activities.
