About the Role

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Hospice Case Manager. This is a full-time position that offers a unique opportunity to work with patients in their homes, providing palliative and hospice care.

Key Responsibilities
  • Manage a caseload of palliative and hospice patients in the home health setting
  • Serve as the clinical liaison for hospice at a neighboring Medical Center
  • Collaborate with healthcare teams to provide comprehensive care to patients
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Provide education and support to patients and their families
Requirements
  • Current RN license in the state
  • Minimum 1 year of experience in hospice care
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Valid driver's license and reliable transportation
