Family Support Coordinator II

As a Family Support Coordinator II at the Children's Home Network, you will play a critical role in delivering family support services to families in need. This involves working with a broad range of community services and natural support systems to provide assessment services, consultations, and quality documentation of services provided. You will participate in the organization's performance and quality improvement efforts, ensuring that families receive the best possible support to achieve their goals.
Key Responsibilities- Maintain a caseload of 12-15 families and provide weekly contact through home visits, office visits, phone contact, or correspondence.
- Work with complex families who may be referred from Child Protection Services (CPI) and/or Eckerd, providing support and guidance to help them achieve their goals.
- Coordinate with community partners to provide outreach presentations and provide education to families on available resources and services.
- Attend at least 2 support group related events annually as assigned, and ensure that families who complete the program achieve their Support Plan goals.
- Ensure that children living with relative caregivers remain outside of foster care one-year post discharge from the program, in accordance with Kinship program expected outcomes.
- Create, implement, and evaluate the effectiveness of each family's Support Plan, and complete assessments, program statistics, and other required documents as required.
- Ensure that families have follow-up linkages into the community once completed program, in accordance with Kinship program expected outcomes.
- Plan, organize, and prioritize responsibilities to ensure adequate timely services to caregivers and timely documentation of services provided.
- Assist families in navigating community resources and addressing barriers, including transportation, advocacy, legal, medical, and mental health needs.
- Meet with Program Supervisor or Program Manager bi-weekly for supervision to review cases, training, and professional development and program development.
- Work within the philosophy and mission of the Children's Home Network, ensuring that files are orderly kept and up to date, and participating in quality assurance reviews of the program services, outcomes, and records.
- Bachelor's Degree from an accredited university or college in Human Services related field, with one year of experience working with children and families. Master's Degree can substitute for the one year of experience working with children and families.
- Valid Florida driver's license with no record of criminal driving offense or license suspension.
- Means of providing job-related transportation and proof of required insurance.
- Ability to work flexible hours, including evenings and weekends.
- Successful completion and maintenance of FDLE clearance, Federal background, and State criminal background check and sexual predator screening.
- Current CPR and First Aid Certification.
