Job Summary

We are seeking a highly skilled and compassionate Field Case Manager I to join our team at Community Health Plan of Washington. As a Field Case Manager I, you will play a critical role in ensuring our members receive the highest quality care and support. Your primary responsibility will be to assess the needs of our members and develop personalized care plans to address their unique challenges.

Key Responsibilities
  • Conduct thorough assessments of members' needs and develop comprehensive care plans to address their physical, emotional, and social needs.
  • Collaborate with healthcare providers, community resources, and family members to ensure seamless care coordination and support.
  • Provide education and support to members and their families to promote healthy behaviors and self-management skills.
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed to ensure optimal outcomes.
  • Document all interactions and care plans in our electronic health record system.
Requirements
  • Bachelor's degree in a related field, such as social work, nursing, or healthcare administration.
  • Minimum of 1 year of experience in case management or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work independently and as part of a team, with a strong focus on collaboration and coordination.
  • Knowledge of community resources and services, with the ability to connect members with relevant support.
