About the RoleAs a Registered Nurse in Home Health, you will provide high-quality patient care in the comfort of their own homes. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities- Conduct comprehensive assessments of patients to identify their needs and develop individualized care plans
- Provide skilled nursing care to patients in their homes, including wound care, medication management, and education
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication
- Document patient progress and outcomes in a timely and accurate manner
- Participate in ongoing education and professional development to stay current with best practices and industry developments
- Current RN licensure in the state of practice
- Current CPR certification
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
