About the Role

We are seeking an experienced Patient Care Coordinator to join our team at Horizon Health. As a key member of our multidisciplinary team, you will play a vital role in ensuring the delivery of high-quality patient care and support.

Responsibilities
  1. Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers.
  2. Facilitate patient flow and ensure timely interventions to prevent relapse.
  3. Collaborate with multidisciplinary teams to coordinate care and address complex needs.
  4. Provide education and support to patients and their loved ones on addiction recovery strategies, medication management, and coping skills.
Requirements
  • Bachelor's degree in human services or a related field.
  • Minimum one year of experience working directly with adolescents or adults in a human service setting.
  • Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
Compensation and Benefits
  • $32,000 - $35,000 per year.
