LPN Responsibilities:
Deliver direct patient care, ensuring personal hygiene and comfort

Monitor and document vital signs, including blood pressure, pulse, and respiratory rate

Gather and record comprehensive health information from patients

Facilitate communication between patients, registered nurses, and physicians

Administer prescribed medications and injections

Conduct blood sugar testing

Perform urinary catheter insertion and management

Assist patients with their personal hygiene needs

Observe and report any changes in patient conditions

Maintain accurate charting and documentation of medical records

Seek clarification from Registered Nurses when necessary

Serve as a role model for support staff

Uphold the confidentiality of patient clinical records.

LPN Requirements:
Valid LPN license in the state

Current first aid and CPR certification

Minimum of one year of experience in a Long Term Care environment preferred

Familiarity with MatrixCare is a plus
