Job Overview: As a Home Health Nursing Specialist at CenterWell, you will play a vital role in providing personalized, comprehensive home care for patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization. Our care teams include nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers – all working together to help patients rehabilitate, recover, and regain their independence.

Compensation: The estimated annual salary for this position is $75,700-$100,300, based on the required skills, qualifications, and location.

Responsibilities: Some key responsibilities of this role include:
  • Providing admission, case management, and follow-up skilled nursing visits for home health patients.
  • Administering ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
  • Conferencing with physicians in developing the initial plan of treatment based on physician's orders and initial patient assessment.

Requirements: To be successful in this role, you will need:
  • Diploma, Associate, or Bachelor Degree in Nursing.
  • A minimum of one year nursing experience, preferably in med-surg, ICU, ER, or acute settings.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
  • Strong organizational and communication skills.
  • Valid driver's license, auto insurance, and reliable transportation.
