Job Title: Community Outreach Specialist

Job Summary:

We are seeking a highly motivated and compassionate Community Outreach Specialist to join our team at Tri-County Human Services. As a key member of our outreach team, you will play a vital role in connecting street and encampment homeless individuals to shelter, services, and housing. Key Responsibilities:

  • Conduct outreach and engagement activities in high-traffic areas, building rapport and trust with unsheltered homeless residents.
  • Assess client needs and provide referrals to appropriate services, including employment counseling, healthcare, and mental health services.
  • Transport clients to shelters or other destinations as needed.
  • Collaborate with the outreach team to develop and implement effective outreach strategies.
  • Provide case management services to clients, including data collection and entry into Client Track and OATS systems.
  • Assist clients with accessing public benefits and assistance as appropriate.
  • Participate in the annual Point in Time survey, which is required by HUD and coordinated by local homeless coalition.

Requirements:

  • Bachelor's degree in a related field, preferably with three to five years of experience in a mental health or addictions treatment setting.
  • Personal or professional experience with homelessness.
  • Basic knowledge of addictions treatment and/or mental health treatment programs and supportive services.
  • Ability to walk long distances and on rough terrain, lift and carry outreach supplies (up to 30 lbs.), and work in a fast-paced environment.
  • Strong verbal and written communication skills, with the ability to work effectively with diverse populations.
  • Knowledge of community resources and human services, with a commitment to increasing housing stability and self-sufficiency among homeless individuals.
