Job Description

We are seeking a dedicated Licensed Vocational Nurse (LVN) to provide personalized care to an elementary student with specific medical needs in Union City, CA. The ideal candidate will have prior school experience and proficiency in assisting students with various medical needs, including Diabetes management, G-Tube feeding, and Tracheostomy care.

Responsibilities:

  • Provide general LVN duties while offering 1:1 care to support the student's medical needs throughout the school day.
  • Assist the student with activities of daily living, medication administration, and other healthcare procedures as required.
  • Monitor and manage the student's condition, including Diabetes management, G-Tube feeding, and Tracheostomy care, adhering to established protocols and physician instructions.
  • Collaborate with school staff, healthcare professionals, and the student's family to ensure a holistic approach to care and support the student's educational and medical needs.
  • Maintain accurate documentation of interventions, observations, and communication regarding the student's health status.

Required Qualifications:

  • Licensed LVN in the state of California.
  • Demonstrated experience in providing care to individuals with Diabetes, G-Tube feeding, and Tracheostomy care.
  • Prior experience in a school setting is highly preferred.
  • Strong communication and interpersonal skills, with the ability to collaborate effectively with a multidisciplinary team.
