St. Vincent de Paul CARES is a non-profit organization dedicated to making homelessness rare, brief, and one-time. We are seeking an experienced Homelessness Prevention Specialist to join our team in Pinellas County, Florida.

About the Role

This role plays a vital part in our mission to rapidly house individuals experiencing homelessness within 30 days. As a Homelessness Prevention Specialist, you will work closely with our multidisciplinary team to assess the needs of clients, develop personalized care plans, and connect them to resources for stabilization and support services. Your primary goal will be to provide timely, equitable, and Veteran-centered care, ensuring that each individual has access to dignified and affordable housing.

Responsibilities
  • Client Assessment and Care Planning: Conduct comprehensive assessments of clients' strengths, limitations, risk factors, and internal/external supports to optimize their ability to access and maintain health care services.
  • Service Coordination: Work collaboratively with clients, community providers, and healthcare teams to ensure seamless service delivery and achieve optimal outcomes.
  • Advocacy and Support: Serve as a resource for education and support, helping clients identify and access credible resources tailored to their needs and preferences.
  • Collaboration and Communication: Regularly consult with team members to address client needs, share knowledge, and drive continuous improvement in our services.
Requirements

We seek candidates with at least two years of experience in a health care or social services area of practice working with Veterans. A Bachelor's level social worker or related field is required, along with basic computer skills, strong organizational abilities, and excellent communication and listening skills. Additionally, you must have a valid Florida driver's license and reliable transportation, as occasional travel may be necessary.
