About the Opportunity

We are seeking an experienced pharmacist to join our institutional pharmacy team at St. Mary's Hospital in Madison, WI. As a clinical pharmacist, you will play a critical role in providing pharmaceutical care to patients, verifying medication orders, and dispensing pharmaceuticals in accordance with federal, state, and local regulations and hospital policy.

Responsibilities and Requirements
  • Ensures safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring, and dose/therapy adjustment as applicable.
  • Documents medication-related plans and interventions in the electronic medical record.
  • Provides clinical consultation, delivering best-practice, evidence-based, and complete information to practitioners, making recommendations, and encouraging safe and cost-effective pharmacotherapy for patients through multi-disciplinary collaboration.
  • Oversees the medication dispensing process to ensure accurate and timely delivery of optimal pharmacotherapy to patients, including supervision of pharmacy technicians and/or interns who support the role of the pharmacist in the drug delivery system.
  • Precepts pharmacy students and residents, as applicable.
  • Works in a constant state of alertness and safe manner.
  • Performs other duties as assigned.
Education and Experience

Pharmacy degree (Pharm.D.) from an accredited school of pharmacy. No experience required.

Physical Requirements

Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs. Frequent sitting, standing, walking, reaching, and repetitive foot/leg and hand/arm movements. Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors. Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc. Frequent keyboard use/data entry. Occasional bending, stooping, kneeling, squatting, twisting, and gripping. Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs. Rare climbing.

Required Professional License and/or Certifications

State of Work: Illinois, Missouri, Oklahoma, Wisconsin. Pharmacist, Registered - Illinois Department of Financial and Professional Regulation (IDFPR). Pharmacist - Missouri Division of Professional Registration. Pharmacist - Oklahoma State Board of Pharmacy. Pharmacist - Wisconsin Department of Safety and Professional Services.
