Job Overview

The Licensed Practical Nurse (LPN/LVN) will be responsible for delivering high-quality patient care in accordance with physician orders and developed care plans. You will work closely with our interdisciplinary team to ensure seamless coordination of care and achieve optimal patient outcomes.

Responsibilities
  • Deliver and document skilled nursing care in compliance with physician orders.
  • Continuously assess patient condition and report changes to supervisors and physicians as necessary.
  • Participate in developing and updating individualized care plans.
  • Take appropriate action based on patient assessment and achieve expected outcomes.
Requirements
  • Current, unrestricted state license as a Licensed Practical Nurse.
  • At least 6 months of hands-on experience in a clinical setting.
  • Current CPR certification.
