Key Responsibilities- Conduct thorough home health assessments on newborns to teenagers, including teaching, monitoring compliance, and performing various treatments.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Provide education and support to patients and their families to promote optimal health outcomes.
- Minimum 1 year of home health experience, preferably in pediatrics.
- Current and unencumbered state licensure as a Registered Nurse.
- Experience with NG or GTube feedings and ostomy care is preferred.
