About the Role
As a Patient Care Coordinator with CenterWell Home Health, you will play a vital role in delivering high-quality, patient-centered care to individuals in their homes. Your primary responsibility will be to develop and maintain relationships with healthcare professionals, hospitals, and skilled nursing facilities to ensure seamless care transitions.
Key Responsibilities
- Conduct facility visits to assess patient needs and develop personalized care plans.
- Collaborate with healthcare teams to ensure effective communication and coordination of care.
- Process physician orders in a timely manner and maintain accurate records.
- Participate in weekly business development meetings to drive program growth and success.
Requirements
- RN/LPN/LVN license.
- Strong knowledge of governmental regulations and Medicare eligibility requirements.
- Excellent customer service, account development, and problem-solving skills.
Preferred Qualifications
- Bachelor's degree in Nursing.
- Minimum of three years of clinical experience.
