About the Role

We are seeking a skilled Primary Care Provider to join our team at Eventus WholeHealth. As a key member of our care delivery model, you will provide primary-care services to adults residing in skilled nursing and assisted living facilities.

Responsibilities
  • Manage all aspects of patient care, including routine check-ups, medication management, and lab reviews.
  • Develop and implement individualized care plans that address the unique needs of each patient.
  • Maintain accurate and detailed records of patient interactions, including documentation of medications, treatments, and test results.
  • Collaborate with facility staff and administration to ensure seamless care delivery and effective communication.
  • Participate in regular quality and compliance reviews to ensure adherence to regulatory standards and Eventus WholeHealth policies.
Qualifications/Education

To be successful in this role, you must possess a Master's degree in Nursing from an accredited institution and a current, valid license to practice medicine issued by the state in which you reside. Additionally, you should have specialization in geriatric, family, or psychiatry.
