Centers for Independence is seeking a skilled Mental Health Case Manager to join our team. As a professional in this field, you will play a vital role in providing care and support to adults living with mental illness.
Job Summary
This is a full-time position that requires a Bachelor's Degree in Social Work, Counseling, or a related Human Services field. The successful candidate will have experience working with adults with severe and persistent mental illness, preferably with dual diagnoses.
Responsibilities
- Develop treatment plans and provide psychosocial rehabilitation, symptom monitoring, and supportive counseling to clients on your caseload.
- Maintain accurate documentation and case files as required by agency policy, government regulations, and applicable codes.
- Coordinate client access to entitlement programs and act as protective payee for Social Security benefits when necessary.
- Provide education and liaison with clients' families and natural supports.
- Assist clients in securing adequate housing and prompting training for necessary skills.
- Responsible for meeting departmental billing quotas, EHR note entry within 24 hours, biannual treatment plans, and other funder-required documentation.
- Attend staff meetings, in-service training seminars, and conferences as required.
Requirements
- Bachelor's Degree in Social Work, Counseling, or a related Human Services field.
- Experience working with adults with severe and persistent mental illness, preferably with dual diagnoses.
- Valid driver's license with reliable transportation and vehicle insurance that meets agency standards.
