Job Summary

The Nurse Practitioner will provide holistic, compassionate care to members and families in collaboration with physicians, staff RNs, and other healthcare professionals. This role involves diagnosing, managing, and preventing acute and chronic illness and disease, as well as promoting wellness.

Key Responsibilities
  • Perform comprehensive health risk assessments on assigned members
  • Work collaboratively with physicians, case managers, social workers, and other healthcare professionals
  • Identify and document chronic disease in adults and develop individualized care plans
  • Prescribe or perform various treatment modalities, including medications, injections, and wellness counseling
  • Refer members to PCP, case management, and disease management programs as necessary
Requirements
  • Minimum 2 years clinical experience as a Nurse Practitioner in geriatric, adult, or family practice setting
  • Proven ability to perform comprehensive, adult/geriatric physical exam
  • Board certification by AANP or ANCC, in Adult, Family Practice, and Geriatrics or Adult-Gerontology Primary Care
  • Current RN and NP licensure in state of practice
Preferred Qualifications
  • Geriatric certification
  • Bilingual (English & Spanish)
  • Knowledge of community resources
  • Previous experience working with behavioral health populations
