Job Summary

Vivage Senior Living is seeking an experienced MDS Coordinator or Registered Nurse to join our team. This is a fantastic opportunity to work in a dynamic and growing organization, leading and coordinating MDS systems in our skilled nursing and long-term care facilities.

Key Responsibilities
  • Leadership and Coordination: Direct, coordinate, and lead MDS Systems, ensuring timely and accurate completion of Medicare A, Medicare B, and other insurance providers' requirements.
  • Clinical Expertise: Perform thorough clinical assessments to determine residents' skilled needs, capturing all resources, and maintaining comprehensive knowledge of MDS and Medicare/Medicaid reimbursement.
  • Collaboration and Communication: Work closely with IDT members (nursing, dietary, social services, restorative nursing, physician services, and rehabilitation team) to ensure seamless information flow and accurate level of skilled need.
Requirements
  • Experience: At least 6 months' experience as a Registered Nurse within a skilled nursing or long-term care facility; PCC (Point Click Care) experience preferred.
  • Licensure: Registered Nurse (RN) license for the state in which you practice or ability to convert to a Colorado License within 30 days of hire.
  • Certifications: Current CPR/BLS certification; COVID-19 vaccine required.
Compensation and Benefits
  • Salary Range: $60,000 - $80,000 per year, depending on experience and qualifications.
