Case Manager Job Description

The Center for Human Development is seeking a skilled Case Manager to join our team at Grace House, a residential treatment program for mothers in early recovery. As a Case Manager, you will provide support services to women with children and/or pregnant individuals in a comprehensive six-to-nine-month residential treatment program.

Key Responsibilities:
  • Provide service coordination and linkages to the community
  • Develop helping relationships through the use of programmatic structure and evidence-based models, including Motivational Interviewing (MI)
  • Connect clients to various community entities and help them find resources for a successful treatment and discharge
  • Facilitate groups for up to 12 women at a time
  • Assist clients in obtaining benefits, housing, and healthcare as needed
  • Provide support and participate in the development, revision, and utilization of relapse prevention and/or safety plans
  • Encourage and facilitate the utilization of clients' natural support systems, as well as recovery-oriented, peer support, and self-help supports and services
  • Assist in data collection and tracking for grant evaluation
  • Maintain and update appropriate treatment plans for residents
Requirements:
  • Bachelor's degree from an accredited educational institution or at least three years of relevant work or lived experience acceptable by MBHP waiver
  • Ability to pass certification for CPR, First Aid
  • Valid driver's license and ability to provide transportation on an as-needed basis
  • Ability to counsel and support clients with co-occurring and/or substance use disorder diagnoses
  • Demonstrated knowledge of trauma-informed care
  • Candidates who are bilingual in Spanish/English are encouraged to apply
Success Factors:

The ideal Case Manager will require empathy and a high level of patience as minor and major setbacks often occur during the recovery process. Candidates with lived experience relative to co-occurring and/or substance use disorder are preferred. Recovery Coaches are welcome.
