About the Role

We are seeking a skilled and compassionate Nurse Practitioner to join our team at Amedisys, Inc. as a Hospice Care Specialist. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to individuals and their families in the comfort of their own homes.

Responsibilities
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Assess patient needs and develop personalized care plans to address physical, emotional, and spiritual well-being.
  • Order and review diagnostic tests, medications, and treatments to ensure optimal patient outcomes.
  • Collaborate with interdisciplinary teams to develop and implement comprehensive care plans.
  • Provide education and support to patients and families to promote self-care and independence.
Requirements
  • Current and unencumbered license as a Nurse Practitioner in the state where services are provided.
  • Minimum of one year experience as a Nurse Practitioner or minimum of one year experience as a hospice Registered Nurse in the last three years.
  • Authority to practice as a nurse practitioner in the state where services are provided.
  • Authority to prescribe as a nurse practitioner in the state where services are provided.
  • Medicare billing privileges as a nurse practitioner.
  • Collaborating physician agreements and insurance requirements as specified by state law.
